    I figured out how to correct the issue. Noticing that the Add-in was "Unloaded" prompted me to check the registry. I eventually found the OPM add in located at:

    HKEY_CURRENT_USER\Software\Microsoft\Office\Outlook\Addins\FileToWSSOutlookAddIn

    And edited the value of LoadBehavior (REG_DWORD) from 0 (Unloaded) to 3 (Load at Startup).

    I hope this can answer some questions for others in the future!

    I completed all of those steps successfully, however, I did not receive an error message. The same behavior occurred. I checked the add-ins and OPM was "unloaded". Here is the setting added through the tutorial, just as evidence I carried out the process.

    The machine in question has been running Office 2013 for several months. Additionally, it has been running OPM for about a month with no issue until recently. I should also mention that there is no error message, the add-in simply turns itself off.
